Your Medical History

When you come in for your appointment, most of you will be asked to fill out a patient update on our IPad.  This is important for a few reasons.  First, although much of your information is indeed the same, there may be minor changes.  Now that we are trying to go paperless, it is important that you verify that the information that we have, was inputted correctly.  Also since we are sending more and more information via e-mail and text message, we need to have both your e-mail and cell phone numbers.  Finally, it is an important part of every medical exam for us to verify your medical history.  This article from the Academy of General Dentistry mentions some of the changes which will require you to fill out a new medical history update.  (The Academy of General Dentistry’s core purpose is to advance the value and excellence of general dentistry through a commitment to the education of general dentists and their patients.
